This shouldn't need such a large introduction. Basically, this FAQ lists all of the HP Memory locations in the game. It should be straight-forward from here on. But if this is your first time playing this sort of game, I'll briefly introduce the concept now.
HP Memory is an item found in the game that permanently raises your maximum HP by a given amount. Two different increments exist and that is 10 and 20. Therefore, picking up an HP Memory 20 will raise your max HP by 20 points. The game has many, limited HP Memory items. Collecting all of them should raise your HP to a maximum of 1000, without power-ups of course. They can be found in the real world or the Wave World. Card Shops also carry 3 HP Memory items so don't miss out on those. Most are found in the Wave World (ER) in Blue Mystery Waves, especially in EM Wave Spaces(ESes).
Since there are a lot of HP Memory items and some are tricky to find, I've made a list of all of them. Here is the meat of the FAQ.

The way I'm listing these goes by the game's naming order. Basically, this means that they'll be categorized by area. It goes: area > room > location. The ESes are generally arranged by area.

Some HP Memory locations are harder to locate that others. I'll include some instructions on the ones I consider a little more difficult to find than the rest.
-Aaron Boreal's Upgrade This job request is only available after the game is completed. After accepting his request, he'll ask you some questions. They're very simple questions but at the end, the wording gets kind of complicated. He claims to give you a Recover 150, but in fact he needs to borrow one of yours to complete his little project. After failing, he just gives you the HP Memory 20 as a reward plus a Blank Card.
-Mess Village Binoculars The binoculars are on the top floor of the observation deck. The far right wing of the floor has two secrets. The binoculars on the left open up a Wave Road if you pay 500 Zennys. The ones on the right have the HP Memory 10.
-Whazzap drum This one might easily be missed. It's one of the drums in the center of the map. They have a pedestal and two drums on the sides. The HP Memory is on the right of the roast.
-Hertz Drawing The Mr. Hertz has a drawing on the far left of the map. Check the description and you will get the HP Memory as well.
-Shepard ES This one was actually my last HP Memory the first time around. It's very simple to miss because the spot where the ES is isn't easily found. The sheep you must analyze is the white one in between the herd of sheep in Messie's Cove. A boy is standing next to it and this ES opens as soon as you arrive there.
-Octy ES Unless you don't look around too often, this HP might be missed. In Loch Mess 2, there is an octopus on the bottom right of the map. The first time you come there, the ES won't be open. After the chapter, check the Loch again and not only will the Octy ES be open, but so will the Crown ES in Loch Mess 1!
-Shop Locations Just in case, here are exact locations of shops with HP Memory in stock:
BIG WAVE Obviously, this is in the BIG WAVE shop. Ken Suther sells this as soon as he opens shop sometime midway through the game.
Shopping Plaza This is the first shop you will encounter in the game. In the Shopping Plaza in Wilshire Hills, there is a Merchant Hertz in the Wave World that sells HP Memory 20. As always, 3 in stock with increasing price. He is on the top floor near the elevator.
Whazzap Lines The Merchant Hertz here is near the middle of the map. The Wave Road there can be confusing at first. Take the left route from the entrance to the ruins. Without taking any warps, you should reach the shop and buy HP Memory.
Bermuda Maze It is not hard to find at all. Upon entering the second time in the story,(after Harp Note) the Merchant Hertz will appear near the entrance and he sells rather expensive HP Memory.
Trans-Dimension This is the last shop in the game. Here, the most expensive HP Memories can be found. The shop is at the left side of the map. You have to beat Dark Phantom IF the first time before accessing this shop. The shop also sells a version-specific Giga Card.

There are questions that tend to come up more than others when it comes to finding all the HP Memories. Here are some answers to common questions.
Q: How come I can't help this person? A: Some people don't have a job request until you are finished with a certain chapter. Advance further in the game and check if they are available.
Q: How come some ESes won't open? A: Just like the job requests, some ESes only become available after a certain point in the game. Some open automatically like the Octy ES in Loch Mess 2.
Q: The Quiz Queen won't give me her quiz! A: To take the quiz from the Quiz Queen, you must finish the quiz given from the previous quiz master.
Q: I'm still missing one more HP Memory. Where is it? A: If you've checked the entire list and the frequently missed locations section, then I might have actually missed it. You'll have to inform me.
Q: What is BMW? A: BMW stands for Blue Mystery Wave. Those are the small blue gems you can find in the Wave World. They don't regenerate like Green Mystery Waves and usually have rarer items.
